    Post BEP scans

    My wife and I were just talking and realized we don't know how long after our son finishes his last round of BEP, scans are done. Anyone know the answer to that? Actually I am sure some of you know so well anxiously wait your response. We are to go see Dr. Einhorn as soon as we've done them and we are trying to figure out when to book the hotel and airline tickets for.

    I had my post chemo CT scan done a few days before my last bleo day which was my final day of chemo. I'm sure some doctors might do it differently. But usually it should be done sometime leading up or a little after the end of chemo. I'm not sure if there's a protocol that is in place for that.

    3/11/14 - Right orchiectomy
    non seminoma - stage IIC 100% embroyal carcinoma
    2 enlarged lymph nodes
    3/31/14-5/27/14 - BEPx3
    lymph nodes decrease in size but still about 3 cm each
    7/16/14 - RPLND - found only dead cancer

      i have mine scheduled for 4 days after my last bleo.When i questioned if it was too early, they said it was normal. I was concerned because i didnt know if it would be ample time for my nodes to return to normal, but they said thats what we are doing.

        We were told to have our ctscan done 4-6 weeks after his last bleo. He finished chemo Nov. 25 and he had his ctscan Dec. 24. I would ask your doctor what he thinks. From what I read, everybody is a bit different.
        9/6/13 Left I/O. 18 year old son diagnosed 9/13/13 Stage 2C. Path report: embryonal and yolk sac with spermatic cord involvement and lymphatic invasion BEP x 3 finished 11/25/13. PC-RPLND with Dr. Foster at IU on 1/21/14. Found only dead cancer! Surveillance as of 1/23/14. All clear as of 5/2018.

            I cannot be certain, but I believe I had one about a week after finishing chemo. My one enlarged node had shrunk, but nt normal, a few months later it was normal, so keep that in mind if the scan is very close to the end of chemo.

              The time frame for a CT versus a Petscan is different. CT can be done very close to finishing Chemo, as to a PETSCAN should be done 4 to 6 weeks out. Chemo has a way of producing false postives with petscan. So the question should be is he getting a CT or a PETSCAN /CT.
              Jan 2012- U/S mass in Left testicle
              Feb 2012- I/O performed to remove cancer
              Mar 1,2012- pathology pure seminoma
              Mar 7, 2012 PET SCAN stage IIa
              April 2012 Mayo clinic carbolite.
              May 2012 carbolite failed, started BEP x3
              August 7th 2012- BEP complete
              April 2013 CT/PET show relapse
              May 2013 RPLND
              Aug 2013 Relapse again Started VIeP x2
              Oct 2013 HDC AUTOLOGOUS
              Dec 2013 HDC completed CT/PETSCAN 1.1 cm x .8 cm right lower lung lobe
              Feb 2014 confirmed false positive all clear FINALLY !
              Jan 2015 1 year cancer free Pet/CT scan
              Jan 2016 2 years cancer free "Pet/CT scan
              Jan 2017 3 years cancer free "Pet/CT scan
              Jab 2018 4 years cancer free "Pet/CT scan, labs, xrays

                Agreed. I had a PET scan 4 weeks after my final Bleo push.

                May 1st: promoted to Stage IIB with two PET active tumors in the para-aortic lymph nodes 2.5 & 2.4 cm
                May 12th: started 3xBEP
                Neupogen during Cycle 2 and 3
                July 8th: Last Bleo shot of Cycle 3 -- chemo completed !
                August 4th: Post Chemo CT/PET scan
                September 4th: Port removed
                Jan 9th 2019: 4.5 YEARS ALL CLEAR !

                  The recommendation for CT scan is during or shortly after the 3rd/final course of chemotherapy. I've had mine 3 weeks after finishing 3rd round of BEP. As far as I know the nodes might be shrinking even 1 month post chemo.
                  07 Aug 2014 - USG showed 5mm mass, bHCG: 6,45 AFP: 4,58
                  08 Aug 2014 - left I/O surgery - NSGCT (70% Mature Teratoma, 30% Embryonal Carcinoma), pT1N0M0, no LVI invasion
                  22 Aug 2014 - Markers after I/O: bHCG: 0,05 AFP: 3,40 - CT scan negative
                  --- on surveillance ---

                  20 Oct 2014 - bHCG: 30 AFP: 8
                  04 Nov 2014 -bHCG: 270 (!) AFP: 26, CT shown 2,5 x 1,5 cm para-aortic lymph node
                  --- recurrence confirmed ---
                  12 Nov 2014 - MRI of brain - clear, re-staged to IIB,
                  3x BEP started, changed to 4x BEP
                  08 Jan 2015 3x BEP finished (bHCG: <0,1 AFP: 4,88), 4th cycle cancelled
                  21 Jan 2015 CT scan clear - para-aortic lymph node shrunk to 0,6 x 0,4 cm

                    They can shrink up to 1 year after chemo.

                    I am 6 months out and the one remaining node is still shrinking.

                      As far as the PET scan, which should really only be used in seminoma patients with a residual mass after chemotherapy of >3cm, the scan should occur at least 6 weeks after chemotherapy because of the false positive risk mentioned previously. I am not aware that there is a set time for the CT. I think 4-6 weeks is reasonable. If you get one immediately afterwards and the tumors appear to be shrinking then what? Do you need another one in a month? I would think giving the chemo a chance to work for a few weeks before imaging is probably more appropriate.

                        All makes sense and thanks to all of you for answering. I think the oncologist wants one month but Dr. Einhorn wrote and said he wants to see our son right after his last round is finished (within that week) but to take the post-chemo scans to IU with us. I guess we will just ask them to do them the week of the final bleomycin and go from there.
