    Advice on University of Chicago for LAP-RPLND

    It has been 3 years since my chemo, but now my CT shows two tumors (they have been there since the beginning) starting to grow again. I am looking at all my options. My oncologist suggested open RPLND, but I was wonder since the two tumors are easy to spot on CT and my markers show no activity if L-RPLND would be a good option.

    Does anyone have any advice on the subject? How about someone to contact at U of C.

    4/28/09 right orchiectomy
    5/11/09 STAGE 3B - AFP 21863, lymphatic invasion, small line on lungs, metastasis with a lymph node larger than 2cm but less than 5cm. No distant metastasis.
    5/26/09 Started 4xBEP
    9/25/09 Markers returning to normal/remaining tumors necrotic
    4/2010 - All clear
    9/2010 - All clear
    4/30/2012 - Remaining tumors showing growth/ AFP/B-HCG in normal range
    7/2/2012 investigating RPLND modified template

    I had my L-RPLND in May with Dr. Eggener at U of C. He is great I loved him and he is the nicest doc I've met. However, I know he mentioned the post chemo L-RPLND is usually not an option but maybe there are exceptions. I would def give his office a call and schedule an appt to discuss the options, he is a smart man and specializes in this stuff, best of luck!!

    I have an appointment today with oncology today here in Durango then I might contact Dr Eggener @ UC. I think you are right though. What I am reading is that L-RPLND might not be the right choice after 4xBEP chemo. I am also going to see if my oncologist has checked with Dr Einhorn - they have in the past. DAVE

    I'd do full open RPLND also. You'll know exactly what's there and you can remove more nodes with the full open, especially after chemo. I'm guessing it's probably teratoma, in which case chemo wouldn't do anything anyways.

    February 2011 - DX Stage IIB, 90% Embryonal Carcinoma, 10% Yolk Sac
    Mar-May 2011 - EPx4 per Dr. Bosl at MSKCC, administered locally in DC
    6-Jun 2011 - post chemo CT shows one lymph node at 1.4cm remaining
    22-Jun 2011 - RPLND with Dr. Sheinfeld at MSKCC, all 51 nodes removed negative
    Dec 2012 - 1.5 YEARS ALL CLEAR!

    They believe the 4.5cm tumor is mostly scar tissue and maybe teratoma since my markers are not elavated and we are 2.5 years post-chemo. L-PRND sounds like it is not a good choice due to location of tumor up against my aorta. My choices seem to be open RPLND or mass excision. Additional chemo is not an option since no cancer is detected.

    My son had an open RPLND with Dr. Eggener on 6/13/12. He was home by the 16th and is recovering very well. The care and attention he received from Dr. Eggener and his entire staff was the best we've experienced on this TC journey.
